We pride ourselves as a service provider that caters for clients with mental health with the focus on constructive steps towards reintegration into the society.
Our services are open to adults and developing young adults with no upper age limit, from all backgrounds, who want to acquire the skills required towards independent living and reintegrate into society.
We also provide a tailored service that allows our clients to learn to live independently within their own homes from 18 years old. The Supported Living Accommodation at Carebridge (UK) will be your new home, and you will be provided with a Tenancy Agreement and the keys to your own personal space/room which can be further customized to create your own unique homely surrounding. Our service users will be treated as clients and the focus will be all about You.
Your Tailored Support Plan.
In addition to your tenancy agreement, you will be part of your tailored support plan (TSP) which will guide us to understand the unique things about you.
Our aim is to encourage you to work towards building yourself, learning to be independent and being actively part of a team and making a positive contribution to our wider community.
Our goal would be to monitor your milestones of progress and provide guidance to help you make better lifestyle choices. You will work with our support workers who will follow your Tailored Support plan to achieve and sustain your agreed set goals.
Your Support Network
CareBridge Limited has a highly skilled and caring support network who are trained to provide support round the clock. Our support network will assign an experienced key worker to you and you will get as much help as you need with the focus being on what you want to do to better equip yourself.
Furthermore, our support workers have direct access to our management team who have the relevant field knowledge and experience as they have been working in the NHS and in private mental health care settings. We also provide selected internal and external training for our team in a bid to stay abreast of current developments in psychiatric and learning disability care as well as continuously understanding the various client needs that evolve over time. All new support workers will work towards completing the Induction Training and QCF framework qualifications, with QCF Level 2 Health and Social Care, being the minimum requirement. .
